This service should be completed for new access, modification to access, and termination of access to the Banner Student/Instruction Module.

Features
Data Stewards will grant access appropriate to the role of the requester. In the "Describe Role" section of the request form, please be sure to thoroughly describe information about the user/requester to ensure Data Stewards assign appropriate access i.e. "AA handling finance", "set-up user like ______", etc.
Getting Started
The Sierra College Computer Use/Ethics/Banner Request Form must be completed and on-file in Human Resources prior to submission of this form. This form should be considered a formal request for access or modification of an existing record in the Student/Instruction Module in the Sierra College Banner database.
