Special Programs - Lending Library Request

Please Read Before Submitting

Request for the use of equipment and/or educational materials from the Special Programs Lending Library. I understand that all requests are processed on a first-come, first-served basis and that submission of a request does not guarantee approval or availability of equipment. If my request cannot be fulfilled, I will be notified via email with additional information or instructions. I understand that priority consideration may be given to students who are actively participating and in good standing with their respective program.

By submitting this request, I acknowledge and agree to the Lending Library Terms and Conditions, including the responsibility to properly care for all borrowed items, return equipment by the end of the semester, and complete a new request for future semesters if continued access is needed. I understand that failure to return borrowed equipment and accessories may result in holds, replacement fees, and/or loss of future borrowing privileges.

FAILURE TO RETURN: If I fail to return the loaned items by the end of the semester or return them in an unsalable condition, I authorize Sierra College to charge the replacement of the items to my student account and/or place a hold on my student account until the debt is paid or the item is returned.

CONDITION UPON RETURN: Loaned materials must be returned in complete and salable condition. This means there is no damage to the item, and all the component parts of the item must be present. I am responsible for any loss or theft of the loaned equipment.
