HDEV/EDU IMMUNIZATION REQUIREMENTS
SB 792 affects all students completing activities in child care settings. You will need to complete the steps listed below to remain in these courses. Students without immunization clearance as listed in the Sierra College Schedule of Classes under each course section will be dropped from the course.
The COVID pandemic has added additional requirements to many placement settings and these are continuing to fluctuate. Employers dictate these requirements. Each instructor of record will communicate with students regarding these requirements, if any, for each course. This is in addition to SB 792 requirements.
1. Contact your former and current health care providers to gather documentation for the following current vaccines OR to make appointments to complete the following (see Sierra College Health Center cost information included at the end of this flyer):
- Measles
- Proof of 2 MMR vaccines after one year of age OR
- Laboratory proof of immunity
- Pertussis
- Proof of Tdap vaccine within the last 10 years and remaining current through the end of the current semester
- Influenza
- Vaccine must be received between August 1 and December 1. (Generally available from September/October through March/April depending on the year.) This immunization may be declined by attaching a written statement to documented immunizations.
- Proof of freedom from tuberculosis no older than 1 year but must remain current through the semester
- For HDEV 10 Students Only:
- Print out a Health Screening Report LIC 503 (see website below) and have it completed by your physician or the Sierra College Health Center. The cost of a physical at Sierra is
$10. This form cannot be completed more than one year prior to the start of the first date of the semester of enrollment. http://www.dss.cahwnet.gov/forms/english/LIC503.pdf
2. Contact the Sierra College Health Center as soon as possible to make an appointment to complete vaccinations OR if vaccinations are complete as listed above, to obtain Early Childhood Vaccine and Tuberculosis Clearance for the course in which you will enroll.
- Please note that if you are enrolling in HDEV 10 this form as well as supporting documentation MUST be completed and presented at the time of your orientation per instructions in Online Course Schedule. Please note health center availability at the end of this document.
3. Compile both your COMPLETED Sierra College Early Childhood Vaccine and Tuberculosis Clearance as well as all the documentation needed to get this clearance and make two sets of copies. Bring these with you to the first class session and submit to the instructor; you will not receive these back, so please keep the originals for your own records. If you do not bring these on the first day of class, you will be dropped from the course.

Flu Vaccine free Blood Draw:
MMR Titers $ 25.00, to show immunity to Measles, Mumps, Rubella
If a student has private insurance, they can get vaccines from their private physician. Most vaccines should be covered by private insurance.
